WAIVER & RELEASE FORM

*Risk & Liability: I understand that sensory play involves inherent risks and materials that may cause minor messes, stains, or accidental damage to clothing or personal items. I acknowledge that incidental physical contact may occur for safety purposes. I release Sensory Celebrations and its staff from liability for injuries, property damage, or personal loss occurring during participation.

*Supervision Required: I agree to remain with and supervise minor children in my care at all times. I understand that Sensory Celebrations is a play experience and does not provide childcare.

*Photos: I understand and acknowledge that photos and videos may be taken during the event and may be used, shared or reshared by Sensory Celebrations for promotional and social media purposes. (Note: We primarily use overhead shots). I am not holding Sensory Celebrations responsible for photos shared by the public.
